Measurement of Plasma Heparin Based on the Potentiation of Anti-Xa

Abstract
It has been accepted that the anticoagulant action of heparin is mainly due to the
potentiation of two of the natural inhibitors present in plasma, antithrombin ¥² and
anti-factor Xa. Heparin therapy is routinely monitored with a clotting assay, such as the
activated partial thromboplastin time(APTT) and the development of chromogenic
substrates has made feasible the selective measurement of heparin-potentiated anti-Xa
or anti-¥± a. a unique clotting method for heparin quantitation based on anti-Xa activity
was also developed and commercially available.
The author compared the results of plasma heparin using APTT, chromogenic
substrate assay for anti-Xa and anti-¥±a, and clotting method for anti-Xa. Heparin
concentrations were classified as being below, within, or above a defined therapeutic
range(TR ; 0.2-0.55units heparin/ml). The considerable discrepancies between heparin
concentrations determined by either APTT or the other methods. Standardization or
recommendation for the optimal method with respect to TR needs to be documented
to assess the individual dose requirement in heparin treatment.
